This product looks really interesting, but - to justify the price - can someone please explain to me how it's better than a nice big, thick pillow?

Softer than a pillow and actually molds to your body better with your body's heat. This is tempurpedic foam - not just "ordinary" pillow foam and thus supports while molding to your body at the same time. I use this and it helps tremendously with joint and back pain. I tried the pillow method first and it doesn't work as well. This is a much better product and is designed specifically for orthopedic usages - unlike pillows.

Does this strap prevent you at all from waking up on your back? I always start on my side and wake up on my back, with my back hurting.

No, it does not prevent you from turning over. When I want to turn over, it wakes me up, have to adjust it and hopefully be able to go back to sleep. I do not use the strap.

Does this pillow separate the knees as well as the ankles?

Yes it does if positioned correctly.

Can I buy a pillow case for this pillow?

I’d be surprised if you could because of the attached elastic straps.
