does this item come with the drain hole in the middle and with a left side pipe holes?

No it just slides over the after you cut the tub

It talk about max tub requirements is there a minimum?

As long as your bathtub does not exceed the maximum tub requirements listed, the product should work on your tub. There are no minimum tub rail width requirements.

What about a door in the "cutout" so I can still use the tub if needed?

There is no door. You will no longer be able to use the tub.. it turns it into a shower only

I don't see the altra low cleancut opening?

It depends on your tub. Our regular guestroom tub made the step-through less than four inches from the bathroom floor.
